CLINICAL EXPERIENCE OF FACIAL NERVE PARALYSIS BY EXTRACRANIAL TRAUMA

Abstract
Facial paralysis follows trauma to extracranial facial nerve results in functional deficit and cosmetic disfiguration of face. In every case, choice of proper management is necessary to restore some degree of facial function.
We present a review of 12 patients with facial nerve transection caused by extracranial trauma and our experience managing this problem from Feb. 1984 to Aug. 1989. The results were summarized as follows.
1) The most frequent etiology of facial palsy caused by extracranial trauma is car accident (58 %) followed by stab wound(17 %), inevitable surgical resection(8 %), iatrogenic surgical injury (8 %), flame bum(8 %).
2) The age range was 5 to 56 years., and the injuries is most frequent in the 4th decade of life(42 %).
3) The total number of involved facial nerve branches were 16 in patients and its distributions were as follows.
Frontal branch(44 %), Zygomatic branch(25 %), Buccal branch(25 %), Marginal mandibular branch(6 %)
4) The two basic types of surgical procedure are used to repair the transected facial nerve. In order of choice, they are(1) end to end anastomosis, 7 cases, (2) interpositional nerve graft, 2 cases. And both surgical techniques proved to be successful in restoring facial function than conservative care in management of facial palsy caused by extracrancal trauma.
